NUMBING CREAM CONSENT FORM
Please read and fill out the consent form below. This is an acknowledgment and acceptance of potential reactions to the numbing cream utilized during the tattoo appointment. By signing this Agreement, you acknowledge and agree to the following terms regarding the use of numbing cream during your tattoo appointment.

Acknowledgment of Numbing Cream Use:
The Tattoo Artist may apply a topical numbing cream to the area being tattooed to help minimize discomfort during the procedure. While numbing creams are generally considered safe for most people, each individual's skin and body may react differently.
Potential Reactions:
The Client acknowledges that the use of numbing cream may cause adverse reactions, including but not limited to:
Allergic reactions (e.g., itching, rash, hives, swelling)
Skin irritation or redness
Numbness or tingling
Burning or stinging sensations
Other unexpected side effects
The Client understands that these reactions are possible and may vary depending on individual skin sensitivity, health conditions, or personal history with topical creams.
No Guarantee of Results:
The Tattoo Artist does not guarantee that the numbing cream will be effective in completely eliminating discomfort during the tattoo procedure. Some individuals may still experience varying degrees of pain or sensitivity during the tattooing process.
Health and Medical History:
The Client agrees to inform the Tattoo Artist of any known allergies, sensitivities, skin conditions, or prior adverse reactions to numbing creams, lidocaine, or similar products. It is the responsibility of the Client to disclose any relevant medical conditions or medications that may affect their skin's reaction to the numbing cream.
Release of Liability:
The Client hereby releases the Tattoo Artist, the Studio, and any affiliated staff members from any and all liability, claims, or damages arising from the use of numbing cream, including but not limited to skin irritation, allergic reactions, or any other medical conditions resulting from its application.
The Client acknowledges that they are using the numbing cream voluntarily and at their own risk, and they assume full responsibility for any adverse reactions that may occur.
Emergency Situations:
In the event of a severe allergic reaction or other serious health issue arising during or after the tattoo procedure, the Client agrees to seek medical attention immediately and notify the Tattoo Artist and/or Studio staff. The Tattoo Artist and Studio will make reasonable efforts to assist the Client in obtaining necessary medical care, but they are not responsible for any medical costs or complications arising from the use of numbing cream.
Informed Consent:
By signing this Agreement, the Client affirms that they have been given the opportunity to ask questions about the numbing cream and the tattoo procedure. The Client acknowledges that they have read and understood the information provided and are voluntarily consenting to the use of numbing cream during their tattoo appointment.
Agreement:
I, the undersigned Client, hereby consent to the use of numbing cream as part of my tattoo session and release the Tattoo Artist and the Studio from any and all responsibility for adverse reactions that may occur as a result of its application. I certify that all information I have provided regarding my medical history, allergies, and previous reactions to numbing agents is true and accurate.
