TATTOO CONSENT FORM

Please read and fill out the waiver below. By signing this form, you are providing your informed consent to undergo the tattoo procedure. Please read this document carefully and ensure you fully understand the contents before signing.

Tattooing Process:

I understand that getting a tattoo involves the use of needles, ink, and other equipment that puncture the skin. I have been informed of the risks associated with this process, including but not limited to infection, allergic reactions, scarring, keloid formation, and complications related to healing.

Health and Medical Disclosure:

I understand it is my responsibility to disclose any relevant medical conditions or medications that may affect the tattooing process or healing (e.g., blood thinners, steroids).

Aftercare:

I understand that proper aftercare is essential to ensure my tattoo heals properly. I agree to follow all aftercare instructions provided by the tattoo artist to avoid complications such as infection, scarring, or fading. I acknowledge that failing to follow aftercare instructions may lead to undesirable results.

Tattoo Outcome:

I understand that the tattoo is a permanent body modification and that its appearance may change over time due to factors such as skin aging, sun exposure, and other variables. I am satisfied with the design, placement, and size of the tattoo before the procedure begins.

Assumption of Risk:

I acknowledge that there are risks involved in receiving a tattoo, including infection, allergic reactions, scarring, and other medical complications. I voluntarily assume all such risks.

No Liability for Pre-existing Conditions:

I acknowledge that the tattoo artist and the tattoo shop are not responsible for any complications that arise due to pre-existing health conditions, skin sensitivities, or reactions to the tattoo materials (e.g., ink, needles, etc.). I understand that it is my responsibility to disclose any relevant health conditions and allergies prior to the procedure.

No Intoxication:

I confirm that I am not under the influence of alcohol, drugs, or any other substances that could impair my judgment. I understand that the tattoo artist reserves the right to refuse service if I appear intoxicated or impaired.

Indemnification:

I agree to release and hold harmless the tattoo artist, the tattoo shop, its owners, employees, agents, and affiliates from any and all claims, damages, or liabilities arising from my decision to receive the tattoo. This includes, but is not limited to, claims for infection, scarring, allergic reactions, or any other adverse outcome.

Photography and Use of Images:

I consent to photographs being taken of my tattoo for the purpose of documentation, promotion, or marketing by the tattoo artist and the tattoo shop. These images may be used in social media, websites, or other promotional materials.

No Refunds:

I understand that all deposits and payments for the tattoo are non-refundable. Once the tattoo process has begun, there will be no refunds.

SIGNATURE

By signing below, I confirm that I have read, understood, and agree to the terms and conditions outlined in this form. I voluntarily consent to receiving a tattoo and accept the risks associated with the procedure. I agree to release Simon Tattooing and the designated tattoo studio from any liability related to the tattooing process. I also understand that I have the right to ask questions about the process and to change my mind at any time before the tattooing begins.
